Infectious complications remain a significant cause of morbidity and mortality after solid organ transplantation (SOT). The implementation of effective preventive strategies has greatly reduced the incidence of opportunistic infections over the last decades, but healthcare-associated infections (and particularly infections due to multidrug resistant pathogens) remain common and have the potential to jeopardize transplant and patient outcomes.

The aim of this Special Issue is to highlight the most recent advances in the prevention and management of viral, bacterial and fungal infections in SOT recipients. We will also want to address important challenges and clinical unmet needs in the management of infection in these patients.

The main topics covered in this Special Issue include the use of new antiviral drugs available for prevention and treatment of cytomegalovirus (CMV) infection, as well as new tools for stratifying the individual risk of CMV reactivation. The role of monitoring for oncogenic herpesviruses will be reviewed, as well as how we may use anelloviruses replication to assess and adjust the overall level of immunosuppression. Regarding bacterial pathogens, the management of difficult to treat infections by new antibiotic and non-antibiotic-based therapies will be discussed. Fungal and parasitic infections remain a life-threatening complication; changing epidemiology and new therapeutic options for these infections will also be reviewed in this Special Issue.
