Abstract
further validation and International collaborations that completeness of 86 registry data with the granularity of electronic health records provide a more comprehensive 87 picture of outcomes. Only through such integrated approaches will it be possible to determine whether 88 the metabolic advantage of simultaneous transplantation ultimately translates into reduced vascular 89 complications, preserved organ function, and better long-term health. The debate between simultaneous pancreas-kidney transplantation and kidney transplant alone is 91 therefore not settled but reframed. The survival benefits once attributed to the combined procedure is 92 less clear under modern analytic methods, yet its metabolic superiority remains unquestionable. More 93 and better evidence is required to fully understand how these dimensions balance over the decades of 94 life after transplantation. Until then, the value of simultaneous transplantation should be appreciated 95 not only through survival curves but also through its potential to transform the daily lives of carefully 96 selected patients.
