Methods: A total of 199 patients receiving LT from January 2019 to March 2022 were enrolled and then randomly divided into a training cohort (n =150) and a validation cohort (n =49). Data were obtained from the electronic medical records. Our primary outcome was postoperative AKI as diagnosed and classified by the KDIGO criteria. A least absolute shrinkage and selection operating algorithm and multivariate logistic regression were utilized to select factors and construct the model. Decision curve analysis (DCA) was applied to assess the clinical application value. Results: Five variables were identified as independent predictors for post-LT AKI, including three pre-operative variables (whole blood serum lymphocyte count, RBC count and serum sodium) and two intraoperative variables (insulin dosage and anhepatic phase urine volume). The above predictors were all incorporated into the nomogram model and programmed into an online calculator. The model showed excellent discrimination with a C-index of 0.817 (95% CI: 0.758-0.876) in the training set. Conclusions: We developed an easy-to-use model to calculate the risk of post-LT AKI. This online calculator may help clinicians identify high-risk patients.
